Purpose of Form: The Vocational Placement Agreement shows where/when you will complete Practice Teaching and that you have permission from the placement organisation to be there. It is important that the form is given to LTi before you commence your placement, so that you are covered for public liability insurance.

Purpose of Form: The Observation and Practice Teaching Log shows the dates and times of attendance, as well as the total number of hours. It is very important that you take the time to complete this log, as it is part of your assessment and is very difficult to complete after your placement has finished. Once you have completed your placement, please give the completed log to your trainer. 

This form is used to record your observation of an experienced teacher. You will need to fill in this form each time you observe a lesson.

The PTF Form is to be used by Supervisors to provide feedback on your teaching performance. You will need to ask your Supervisor to complete this form each time that you teach a lesson. This form will form a part of your assessment. Please give your completed PTF Forms to your trainer. 
Hint: If your practice teaching situation will be the same each time you teach (e.g. the learner profile, supervisor’s details, course), you can create a template for yourself by writing details that will be the same for each practice teaching session on the 1st page of the Practice Teaching Feedback form . Then you can photo copy that page, so that you can write the details that will be different (e.g. day, date, and time of class, PTT being fulfilled in this lesson) without having to fill out the entire form again.
